My template and hotkey map for consistent notes

I’m Wyatt. Here’s the small, repeatable setup I use to keep notes consistent across providers while staying HIPAA‑ready. Template baseline

  • Build one base template: CC, HPI, Meds, Allergies, ROS, Exam, Impression, Plan, Follow‑up.
  • Save provider‑specific copies with fixed headings and placeholders. Keep the order identical across all.
  • Lock styles so fonts, spacing, and headers don’t drift. Simple naming
  • Audio: YYYYMMDD_provider_patientID.wav
  • Draft: YYYYMMDD_provider_patientID_draft.docx
  • Final: YYYYMMDD_provider_patientID_final.docx HIPAA habits
  • Full‑disk encryption on the workstation. auto‑lock at 5 minutes.
  • Headphones only. no smart speakers in the room.
  • Use approved storage only, and get a business associate agreement before any cloud use. Pedal and hotkeys (Express Scribe + Word)
  • Pedal map (Infinity IN‑USB2): - Center: Play/Pause - Left: Rewind 2s - Right: Fast‑forward 2s
  • To set: Express Scribe > Options > Controllers > Configure > assign Rewind 2s, Play/Pause, Forward 2s.
  • Keyboard: - F7 spellcheck - Shift+F5 jump to last edit - Ctrl+F find - Ctrl+Shift+L apply list style Daily pass
  • Start each session with a fresh template copy, then paste dictation in order.
  • Use the same placeholders and abbreviations for every provider. I run this first thing at 6 am and put a thin rubber mat under the pedal so it stops sliding under the desk.

How are you triggering sections without breaking flow? I lean on text expanders over EMR templates. For example: . cc → ‘Chief Complaint:’ . hpi → structured prompts.
