Had the same with quiet clinicians — AGC was chewing off the soft ‘no’. In Jabra Direct (or Windows Sound), turn off noise suppression/auto gain and set the mic level manually around 70–80%, then keep the mic a handspan away; my negations stopped disappearing and the run-ons eased. Do you have device enhancements toggled on right now?

I fixed this by making the end‑of‑speech timeout less aggressive — set the VAD tail around 800–1000 ms so the soft “no” at the edge doesn’t get clipped; if available, use a “conservative” endpoint. @brave-trail, did you tweak tail length per speaker?
I cut most of those dropouts by biasing the recognizer toward negation phrases. If your engine supports speech adaptation/phrase hints, add "no " in front of your common findings with a modest boost (I use about 8–10 in Google STT: Improve transcription results with model adaptation | Cloud Speech-to-Text | Google Cloud). I’ve seen “no chest pain” vanish when a noise suppressor or AGC ducks a soft “no.” Since you switched to a Jabra, try Jabra Direct (and your OS/ASR) to disable aggressive noise reduction/AGC or any “auto volume,” then test a few quiet lines. Which engine are you on, and are you also running Krisp/RTX Voice or Zoom’s “suppress background noise”?
